Understanding Piriformis Syndrome
Piriformis syndrome occurs when the piriformis muscle a small, flat muscle located deep within the gluteal region becomes irritated, tight, or inflamed, causing it to compress or irritate the sciatic nerve.
The piriformis muscle plays a vital role in lower body movement, assisting in lateral rotation of the hip and stabilising the hip joint during walking and running. Because the sciatic nerve passes underneath and in some individuals, directly through the piriformis muscle, any spasm or muscular hypertonicity can directly impinge upon the nerve.
“The piriformis muscle is a muscle that sits deep within the hip, and it actually runs over the top of the sciatic nerve,” Dr. Nash explains.
“So when you end up with irritation within that muscle, or tearing, or the muscle’s really tight, that could then potentially impinge onto the sciatic nerve and create pain down the leg.”

Piriformis syndrome typically manifests as local tenderness in the buttock, accompanied by radiating symptoms down the back of the thigh and leg.
“Piriformis syndrome can also be locally sore right across the back of the buttock, and be quite tender to sit down, very sore to move around,” says Dr. Nash.

“Piriformis syndrome is one of the biggest causes of sciatica,” states Dr. Nash. “A lot of people, when they come in with sciatic pain, would think it’s a disc bulge or something to do with their low back.”
While a lumbar disc herniation compresses the nerve root at the spine, piriformis syndrome represents an entrapment of the nerve further down its path in the gluteal space. A systematic review published in the European Spine Journal found that the features most consistently reported in piriformis syndrome were buttock pain, external tenderness over the greater sciatic notch, pain aggravated by sitting, and pain increased by movements that place the piriformis muscle under tension. The same review noted that distinguishing these patients from those with disc herniation or spinal stenosis remains an open research question (Hopayian et al., 2010).
“So piriformis syndrome can be quite debilitating, often confused with disc bulges and other irritations from the low back,” notes Dr. Nash. An updated systematic review by the same lead author reached a similar conclusion, describing piriformis syndrome as definable by a quartet of symptoms and signs while noting that the accuracy of the individual physical tests used to identify it has not yet been established (Hopayian & Danielyan, 2018). Accurate clinical evaluation therefore relies on a thorough history and a combination of assessments rather than any single test, in order to pinpoint the site of compression.
