Understanding neck adjustments

When people talk about having their “neck cracked,” they are usually referring to a chiropractic adjustment (or manipulation) of the cervical spine. The “crack” or “pop” heard during an adjustment is a phenomenon known as cavitation the release of gas bubbles from the synovial fluid within the joint space due to a rapid change in pressure.

At Our Chiro Brisbane, we understand that social media often portrays these treatments as loud, aggressive, or dramatic. However, the reality of a clinical setting is much different.

“It’s never as big as you see on social media,” explains Dr. Nash. “It’s usually quite small in comparison to that. At our clinic, we use objective and peer-reviewed treatment methods, which is usually quite gentle and nothing as dramatic as you see on social media.”

The importance of clinical screening

Safety is our absolute priority. Before any treatment is administered, a comprehensive assessment is performed to ensure that chiropractic care is the right path for the patient. This process begins with a detailed look at your health history and physical condition.

“Firstly, when you come into the clinic, we’ll take a full medical history and that’s really important to guide us in what we’re going to do,” says Dr. Nash.

This screening process allows us to identify any “red flags” or contraindications. If a patient’s condition requires further investigation, we do not hesitate to use diagnostic tools. Dr. Nash notes:

“The next step, we will make sure that if we need to refer you to any imaging, whether it be X-ray, CT, or MRI, that we would then do that.”

Is it safe to have your neck cracked?

Research consistently shows that when performed by a trained professional, cervical manipulation is a safe and effective intervention for mechanical neck pain and certain types of headaches. A study published in the Journal of Manipulative and Physiological Therapeutics found that chiropractic care is associated with high levels of patient satisfaction and low rates of adverse events compared to many common medical interventions for pain. “Just remember the chiropractor will tailor the treatment to your individual needs,” Dr. Nash reassures. “They’re very highly trained and if you have any concerns whatsoever, just ask them and I’m sure they’ll allay your fears.”

To ensure the highest level of safety, Dr. Nash emphasises that practitioners often work within a multidisciplinary framework. “We would refer you to another specialist or medical specialist if that was warranted as well,” he adds.

Different treatment styles and techniques

One of the biggest misconceptions is that a chiropractor must “crack” your neck to provide relief. In reality, we have a wide variety of tools at our disposal. If a patient is uncomfortable with manual manipulation, we simply use a different approach.

“If a patient doesn’t like having the clicking or mobilisations done, we can use a different little tool such as an activator or we can just do gentle mobilisations around the neck,” explains Dr. Nash.

These alternative methods, such as Dry Needling or gentle Chiropractic mobilisations, aim for the same result: restoring movement to the spine. As Dr. Nash puts it:

“All of these have the same effect and we’ll get that biomechanical movement back to the cervical spine.”

The training behind the care

Chiropractors in Australia undergo rigorous university education to ensure they can provide safe and effective care. This involves a deep dive into anatomy, physiology, neurology, and radiology.

“The training that we have is a 5-year degree,” Dr. Nash highlights. “I have a bachelor’s and a master’s degree. So, we are very well trained in doing these little procedures as well.”

This extensive training allows the chiropractor to apply the precise amount of force at the exact angle needed, making the procedure much more controlled than it may appear to an untrained observer.

Q1: What actually makes the “cracking” sound? A: It is simply the release of gas (mostly nitrogen and carbon dioxide) from the joint fluid when the joint space is opened quickly. It is not bones rubbing together.

Q2: Will I be forced to have my neck manipulated? A: Absolutely not. We always discuss the treatment plan with you first. If you prefer low-force techniques without the “clicking” sound, we can use tools like an Activator or gentle mobilisations.

Q3: Can a neck adjustment help with my headaches? A: Yes, many headaches are cervicogenic (originating from the neck). Improving the movement of the cervical vertebrae can often reduce the frequency and intensity of these headaches.

Q4: Is there anyone who shouldn’t have their neck cracked? A: Patients with certain conditions like severe osteoporosis, certain vascular issues, or acute fractures are not candidates for manual manipulation. This is why our initial physical exam and history are so thorough.

Q5: How will I feel after the treatment? A: Most patients feel immediate relief or increased range of motion. Some may experience mild soreness for 24 hours, similar to the feeling after a workout, as the muscles adjust to the new movement.

Q6: Are the techniques the same as what I see on TikTok? A: No. Social media “cracks” are often performed for “shock value.” Clinical chiropractic adjustments are specific, high-velocity but low-amplitude movements tailored to your specific anatomy.

Also, we would refer you to another specialist or medical specialist if there if that was warranted as well. Um, so once we’ve compiled all that, we would then work out the best treatment style for your cervical pain or movement. Now, that can be a manual one, which usually involves the cracking or clicking. So, it’s never as big as you see on social media. Um, it’s usually quite small in comparison to that. Uh at our clinic, we use objective and uh peer-reviewed treatment methods, which is usually quite gentle um and nothing as dramatic as you see on social media.
